CAL’s Forms and Templates is an online store with legal forms and templates created by Chicago Advocate Legal attorneys for use in the State of Illinois.

We created this site to make legal forms and templates simple and accessible for regular people who are representing themselves in court or handling legal matters without a lawyer.

Every form you’ll find here has been created by a licensed attorney with experience in Illinois law. We’ve used these same forms in real cases, and they’re written to meet the specific requirements of Illinois courts.

  • Open Microsoft Word. Under the File tab, Select Options. Select the Customize Ribbon tab. Click the check box to add Developer to your Main Tabs.

    Once you’ve added the Developer tab to your Main Tabs, select the Developer tab. Select Add-ins. Go the Store and add the Doc Variables Add-in.

  • Yes, all of our forms are downloadable in a .word format and can be edited and customized without the use of the Doc Variables feature.

  • It depends. Our legal forms are designed by lawyers, but are meant to be completed without the assistance of a lawyer. However, in certain cases, especially complex and highly contested cases, it may still be beneficial or even necessary to retain an attorney for your case.
